Thymalin is a peptide that has been studied for decades in Russian bioregulation research. What is Thymalin?
Thymalin is a peptide complex originally obtained through mild acid extraction from calf thymus. The active component, later produced synthetically, is the dipeptide Glu-Trp (L-glutamic acid-L-tryptophan), a very small peptide with preclinically reported immunomodulatory effects.
The thymus is the central gland of the adaptive immune system. It is responsible for the maturation and selection of T lymphocytes. As we age, the thymus shrinks (thymic involution), a process that already begins during puberty and is linked to age-related immune decline (immunosenescence). Thymalin is studied preclinically in relation to this process.
The idea behind Khavinson's bioregulation theory: peptides isolated from specific organs are thought to support the function of those organs, among other things through a postulated effect on gene expression. This epigenetic hypothesis originates from Khavinson's work and has not been broadly confirmed outside this group.
Why is Thymalin studied?
Key studies
Khavinson and Morozov (2003, Neuroendocrinology Letters 24(3-4):233-240): This long-term observational study examined the geroprotective effects of Thymalin and Epithalamin (pineal gland peptide) in 266 elderly individuals over 6-8 years; the peptides were administered during the first 2-3 years. The authors reported mortality roughly half that of the control group (the authors report a 2.0-2.1-fold reduction for Thymalin), along with improvements in immune, endocrine, and cardiovascular parameters. Important: these results come from Khavinson's own research group and have not been independently replicated internationally.
Anisimov et al. (2001, Mechanisms of Ageing and Development 122(1):41-68): This animal study examined the effect of synthetic thymic and pineal peptides on biomarkers of aging, survival rates, and spontaneous tumor incidence in female CBA mice, and reported, among other findings, a reduced spontaneous tumor incidence. Such preclinical lifespan findings come from animal models and are not evidence of an effect in humans.
Khavinson et al. (1998, Journal of Neuroimmunology): This article described the isolation of the immunomodulatory dipeptide Glu-Trp from Thymalin using RP-HPLC and its action as an immune corrector in pharmaceutical practice.
Mechanisms of action
Several immunological mechanisms have been described in preclinical investigations:

T-cell differentiation: In cell and animal models, Thymalin was associated with promoted maturation and differentiation of T lymphocytes in the thymus, including the CD4+ (T-helper) and CD8+ (cytotoxic T-cell) populations.

Postulated epigenetic effect: According to Khavinson's bioregulation theory, Thymalin is thought to act at the chromatin level and influence the expression of age-dependently regulated genes. This is a hypothesis originating from this research group and has not been broadly confirmed outside it.

Cytokine modulation: In animal models, Thymalin influenced the balance of pro- and anti-inflammatory cytokines. In aged organisms, this balance typically shifts toward chronic inflammation ("inflammaging"); in animal models, this shift was partially attenuated.

Thymus function: Effects on markers of thymus function and involution have been reported in animal models; this does not constitute evidence of a clinical effect.

Pineal-thymus axis: Khavinson's work describes a connection between the thymus and the pineal gland. In the (not independently replicated) long-term studies, the combination of Thymalin and Epithalamin was reported to have a stronger effect than the individual peptides.

Clinical studies (Khavinson): In the long-term studies, Thymalin was administered in cycles, typically 10 mg daily over 5-10 days, followed by breaks of several months. This cyclic approach is characteristic of bioregulation therapy.
Animal models: Anisimov et al. used various dosing schedules in mice, including long-term cycles spanning months.
Community reports: In unvalidated anecdotal reports, amounts on the order of 5-10 mg over short cycles of 5-10 days are mentioned; these figures are self-reported and not a recommendation.
The study figures come from the published literature; the community figures are unvalidated self-reports. None of these figures constitutes a dosing recommendation, and there is no EU-approved treatment regimen for humans.
